Rep. McNally Announces $440K in Grant Funding to Mahoning County Health Department Pathways Community Program

COLUMBUS — State Rep. Lauren McNally (D-Youngstown) today shared that the Ohio Commission on Minority Health (Commission) has awarded the Mahoning County Health Department $440K in grant funding for the continued implementation of their Pathways Community HUB Model. This program’s goal is to prevent infant mortality and to improve birth outcomes.
“I am so proud of the Pathways program and how it is changing the lives of people in Mahoning County. This program is helping so many expecting mothers and giving them the care that they need,” said Rep. McNally.
The Pathway Community HUB Model is a nationally certified, evidence-based care coordination model. This model connects certified community health workers with high-risk mothers and coordinates appropriate and timely prenatal clinical care. Community health workers also address education, employment, housing, behavioral health, and support to other essential services. This model has shown measurable improved birth outcomes and has been implemented in 12 Ohio counties and 15 states.
The Ohio Commission on Minority Health was an early investor in the Pathways Community HUB Model, partnering with other state agencies and community based organizations to improve birth outcomes for all populations. Their goal is to close the gap in Ohio’s black infant mortality rate. The Commission funding supports the implementation of models designed to improve health care accessibility, prevent chronic diseases and conditions, reduce emergency room use, and demonstrate a return on investment.
